Community Support Services Unit (CSSU)/Case Management CSSU provide services and support in the community, in the home and in the office. Case management activities include assessment, planning, monitoring, counseling and assistance with problem solving, skill training, advocacy, money management, medication management and linkage to other necessary services.
Case management is available at all three MHCCI mental health centers: MHCCI-Lincoln, MHCCI-Jacksonville and MHCCI-Springfield.
MHCCI’s case management program provides intensive community-based services to adults diagnosed with severe and persistent mental illness. Services include outreach, assessment, treatment planning, linkage, referral, counseling, skills training and support.

Life Skills A program designed to identify and address skill deficits in the areas of cooking, cleaning, hygiene, personal safety, etc. Services are provided within the individual’s residence and the community.
Assertive Community Treatment (ACT) ACT is a specialized case management program which emphasizes a ‘team approach’ to treatment. Services are provided to eligible individuals up to five times per week by a team of professionals from diverse programs. The ACT team provides community-based support services to assist individuals in maintaining community living.
Projects for Assistance in Transition from Homelessness (PATH) PATH eligible individuals are those who are mentally ill and homeless or at imminent risk of becoming homeless. PATH team members assist individuals in locating and maintaining safe and affordable housing alternatives.
